Diagnostic correlation between cervical cytology and biopsy in cervical lesions
Abstract
Cervical cancer is a global public health problem, especially in low- and middle-income countries such as Venezuela. Although the global incidence has decreased thanks to screening and vaccination programs against Human Papillomavirus (HPV), in Venezuela it continues to be one of the leading causes of morbidity and mortality among women. With the aim of correlating the diagnosis of cervical cytology and cervical biopsy in patients treated at the cancer prevention clinic from January 2020 to December 2024, a descriptive, cross-sectional, observational study was conducted. A total of 2,163 cytologies were performed, of which 122 patients had cervical biopsies, the latter being the samples analyzed in this study. The mean age was 43.3±12.6 years, with the highest frequency between the ages of 36 and 55. Of the total cytologies, 4.06% showed some type of abnormality, the most frequent being Low-grade Squamous Intraepithelial Lesion (LSIL); in biopsy, 50.82% showed pathological lesions and 18.03% corresponded to invasive carcinoma. The correlation between conventional cervical cytology and cervical biopsy reported a sensitivity of 85.48%, specificity of 41.67%, positive predictive value of 60.23%, and negative predictive value of 73.53%. As a result, cervical cytology maintains diagnostic performance in our setting. Ideally, standardization of auxiliary tests such as colposcopy and typing tests is recommended in order to increase its specificity and reduce unnecessary procedures.
