Importance of preoperative immunohistochemistry in the therapeutic management of patients undergoing surgery for early breast cancer
Abstract
Introduction: Breast cancer is among the most prevalent in the world, being responsible for a large part of the causes of death from cancer. It is a heterogeneous disease where tumor diversity occurs at the molecular level. The lack of accessibility to genetic testing has led to the use of immunohistochemical characteristics as a way to classify breast cancer. However, in some areas of our country, immunohistochemistry seems to be difficult to access, causing that the patient's immunohistochemical profile would not be available prior to surgery in some cases, preventing a correct indication of neoadjuvant therapy in those cases that require it. .
Objective: To determine if the knowledge of the preoperative immunohistochemistry would have modified the therapeutic strategy according to international guidelines.
Materials and Methods: We proceeded to design a transversal descriptive study where 168 patients diagnosed with breast cancer were evaluated, with clinical T less than or equal to T2, who underwent surgery with axillary exploration as the first therapeutic strategy. In every case the immunohistochemical profile of the tumor was unknown prior to surgery.
Results: 134 patients (79.76%) had tumors with positive estrogen and progesterone receptors and were Her2 negative, 18 patients (10.71%) had triple negative tumors and 16 (9.5%) had HER2 positive tumors (10 triple positive, 6 Her2 enriched).
Conclusions: In the triple negative and HER2 immunohistochemical subtypes, the vast majority of patients (72.2% and 87.5%, respectively) had an indication for neoadjuvant treatment according to the NCCN 2022 guidelines and instead underwent surgical treatment as the first therapeutic approach.
Biography
Camila Ramundo is an Argentinian gynecologist and breast specialist, with a focus on breast cancer. She completed her medical training at the National University of La Plata (Argentina) and her residency in gynecologic surgery at Clínica Pueyrredon in Mar del Plata, where she currently works as a breast disease specialist. She holds a postgraduate degree in Mastology and completed a clinical rotation in Breast Surgical Oncology at the MD Anderson Cancer Center (Houston, USA). She is part of the multidisciplinary breast team at Clínica Pueyrredon and also works as an advisor in cancer genetics.
