Empowering birth: Assessing the impact of antenatal education on Indian mothers

Abstract

The National Family Health Survey-5 (NFHS-5) highlights critical issues concerning childbirth and breastfeeding practices in India. Approximately 13% of births occur prematurely, and around 17% of newborns are reported to have low birth weight. Alarmingly, maternal exposure to PM2.5 air pollution during pregnancy is associated with a 40% increased risk of low birth weight and a 70% higher likelihood of pre-term delivery. With respect to breastfeeding, the data reveal a positive trajectory in exclusive breastfeeding (EBF) rates, which increased from 31.3% in NFHS-4 (2015–16) to 43% in NFHS-5 (2019–21). Despite this improvement, over half of newborns experience delays in the initiation of breastfeeding. Factors positively influencing EBF practices include mothers from Scheduled Tribes and those delivering in public health facilities. Conversely, EBF rates are negatively affected by caesarean deliveries, low birth weight, and initiation of breastfeeding delayed beyond one hour post-delivery. These findings underscore an urgent imperative to strengthen antenatal care and maternal education, encourage the early initiation of breastfeeding, and sustain exclusive breastfeeding. Additionally, addressing environmental hazards and systemic healthcare deficiencies remains essential to improving neonatal health outcomes in India.

Shivaranjani is a Women’s Health Physiotherapist, Department of Physiotherapy, Panimalar Medical College Hospital and Research Institute, Tamil Nadu, India. She is a dedicated Women’s Health Physiotherapist with over six years of experience in both clinical practice and academic training. She holds a Master’s degree in Physiotherapy in Obstetrics and Gynecology from The Tamil Nadu Dr. M.G.R. Medical University and is currently pursuing her PhD at Garden City University, Bangalore. She is a certified Lactation and Infant and Young Child Feeding Counsellor (BPNI, Delhi), Certified Childbirth Educator and Pelvic Floor Rehab Specialist (WOW IIPRE), as well as a Certified Fitness and Aerobic Training Specialist (ISST, Pune). Her work is driven by a passion for advancing women’s health and empowering mothers through evidence-based physiotherapy practices. She is also an active member of the Indian Association of Physiotherapists (Reg. No: 61993), and her current role includes clinical services and academic contributions at Panimalar Medical College Hospital and Research Institute.
