Improving compliance with VTE risk assessment in pregnant and postnatal women in King Fahad Hospital- Albaha

Abstract

Background: Venous Thromboembolism (VTE) is a leading cause of maternal morbidity and mortality in pregnancy and the puerperium. Despite the availability of national guidelines, gaps remain in consistent risk assessment and thromboprophylaxis.

Aim: To improve the quality of VTE risk assessment, counseling, and prophylaxis for women during the antenatal and postnatal periods, with a focus on patients attending the outpatient antenatal clinic and those followed in the virtual postnatal clinic.

Methods: A baseline audit was performed to assess documentation of VTE risk assessment and use of appropriate thromboprophylaxis. Interventions included stat education, integration of risk assessment checklists into the OPD workflow, and reinforcement of follow-up through the virtual postnatal clinic.

Results: Baseline data revealed suboptimal compliance with VTE risk assessment and prophylaxis, especially among high-risk women. Following interventions, documentation rates and correct prescribing improved significantly in both antenatal and postnatal patients. Counseling regarding VTE warning signs was also more consistently documented, including during virtual consultations.

Conclusion: Targeted quality improvement interventions in both antenatal OPD and virtual postnatal clinics can substantially improve adherence to VTE prevention guidelines. Sustaining this improvement requires ongoing stat education, standardized assessment tools, and the integration of virtual clinic follow-up into routine maternal care.
